New health provider claim adjustment form

A new form is available for health providers to request adjustments to DVA claims where the original date of service is less than 2 years old. 

The new Application for the Department of Veterans’ Affairs claim adjustment form (DB021) should be used when a previously submitted DVA claim requires correction or updating. 

Available on the Services Australia website, providers can use this form to request adjustments where:  

  • information was missing from the original claim 

  • an incorrect item number was claimed  

  • claim details need to be adjusted 

  • another amendment is required to ensure the claim is processed correctly.  

The DB021 form can be used for claim adjustments relating to: 

  • allied health services 

  • medical or specialist services 

  • hospital services  

  • community nursing services 

  • nursing home services. 

The introduction of a standardised form replaces informal claim adjustment requests submitted on practice letterheads and helps avoid confusion with the Voluntary acknowledgement of incorrect payments (MO057) form, which is used for recovery-related matters.
