CVC Eligibility Tool
This tool is designed to guide General Practitioners through the eligibility criteria that must be assessed for a Veteran Card holder being considered for enrolment in the Coordinated Veterans' Care (CVC) Program.
If you determine that a patient is eligible to participate, you must also conduct an in-person medical and health assessment, ensure that the mandatory written 'Consent to Participate in the Coordinated Veterans' Care Program and Privacy Collection Notice' is completed and develop a comprehensive care plan before enrolment can proceed, as outlined in the Notes for Coordinated Veterans' Care Program Providers.
