How to find your accepted conditions on MyService

MyService allows you to retrieve a list of your accepted conditions anytime and anywhere you have internet access. You can refer to this information yourself and share it with health providers when seeking treatment and services.

In MyService, you can both print or save a PDF list of your accepted conditions to email or share with your health provider if you choose. This gives you a clear record to refer to when discussing your care.

  1. Set up a MyService account if you don’t already have one. You can do this through myGov using an activation or linking code. Instructions are available at How to set up a MyService account.
     
  2. Sign in to your MyService account.
     
  3. Navigate to your Cards screen by selecting either:
     
    • Your information and then Accepted conditions, or
    • Your card and then View card details.
       
  4. On the Cards screen, you will find your accepted conditions listed within the Conditions listed section. Each condition shows the legislation it was accepted under and the date of effect.
     
  5. You can then either save the list as a PDF to send by email or print it to provide a physical copy to your provider. 

If you have your Veteran Card in your myGov wallet, you can select Manage your card from within the wallet. This will take you to the MyService Cards screen where your accepted conditions are listed.

If you are unable to access MyService, you can request a Veteran Card – List of Covered Conditions letter by calling the Veteran Access Network on 1800 VETERAN (1800 838 372) or emailing generalenquiries@dva.gov.au.
