Have your say on improving DVA’s Coordinated Veterans’ Care Program

The Department of Veterans’ Affairs (DVA) has engaged Abt Associates to evaluate the Coordinated Veterans’ Care (CVC) Program.

The CVC Program uses a proactive approach to strengthen participant health outcomes and improve quality of care.

The program is for eligible Gold Card holders with one or more chronic health conditions and White Card holders with an accepted mental health condition who have complex care needs - making them at risk of unplanned hospitalisation.

The evaluation will measure program benefits, including clinical outcomes and wellbeing impacts. The evaluation will also explore options to reshape the CVC Program to better meet the chronic disease management and mental health needs of current and future veterans.

Abt Associates will consult with a broad range of primary care providers who currently help deliver the CVC Program, including general practitioners, practice nurses, and practice managers. Consultation is expected to occur from early 2024 and remuneration is available to providers and practices that choose to engage in online interviews.
