Personal information is information or an opinion about an identified individual, or an individual who is reasonably identifiable. Personal information includes an individual’s name and contact details and may include opinion-based data.
Sensitive information is a subset of personal information. It includes information or an opinion about your racial or ethnic origin, political opinions, religious beliefs or affiliations, philosophical beliefs, membership of associations or unions, sexual orientation or practices, criminal record, or health, genetic or biometric information.
You do not have to provide any sensitive information in your application. By choosing to include sensitive information in your application, or when verifying your identity where relevant, you consent to the department collecting this information.
Who is collecting your personal information?
Your personal information is being collected by the Commonwealth of Australia, represented by the Department of Veterans’ Affairs (ABN 23 964 290 824) (‘The department’).
Purpose of collecting and using your information
Your personal information provided in this Application Form is collected for the purposes of processing your request under the Freedom of Information Act 1982.
If you do not provide some or all of the personal information requested, the department may be unable to process your request. Where possible, the department will make then applicants aware of these limitations.
If you are making a request on behalf of someone else, please ensure you have appropriate authority to act on their behalf and provide that person with a copy of this notice. Do not submit this form on behalf of another person without appropriate authority to do so.
Disclosure of your personal information
In general, we will not disclose the personal information you provide except in the course of processing your request. This may include disclosing your personal information to contracted service providers.
If you request documents that include personal information about other people, we may need to consult with those people before deciding whether we can disclose those documents to you. When we consult with other people about your request, it may be apparent to those people that you have made a request, even though we will not usually disclose your identity.
If you seek external review of a decision by the department, we may need to disclose personal information about you to the external review body such as the Office of the Australian Information Commissioner.
Your personal information may also be disclosed to other parties with your agreement, or where it is otherwise permitted under the Privacy Act or other legislation.
Your personal information is unlikely to be disclosed to overseas recipients.
