Updated RAP order form for ‘Low Vision’ products

The Department of Veterans’ Affairs (DVA) has revised the Rehabilitation Appliances Program (RAP) order form for low vision products after consultation with health advisers and other stakeholders.

The main changes are:

• Revised RAP order details table;
• Addition of a closed circuit television (CCTV) request assessment which was previously embedded in, and now removed from, the RAP National Guideline on CCTV; and
• Clearer positioning to choose a DVA-contracted supplier.

Assessing health providers should ensure the most up to date RAP Forms are being used.
