How to find your accepted conditions in MyService. From your MyService homepage, navigate to the Cards screen. Go to 'your information' on the homepage and select 'accepted conditions'. Or in the 'your card' section, select 'view card details'. Find your list of accepted conditions. Just select 'conditions listed' to see the conditions listed on your card. Share your list of conditions. Select 'print this page' and save to your device. You can email the list of saved conditions to your provider or show them a printed copy during your appointment.
