Updated order form for RAP Mobility and Functional Support Products

The Department of Veterans’ Affairs (DVA) has updated the Order Form for Mobility and Functional Support Products under the Rehabilitation Appliances Program (RAP).

The main changes include:

  • A section to outline specific clinical conditions that necessitate the supply of the item
  • A reminder to attach the D1323 Authority to Install form for home installations/modifications
  • Request to ensure all information is current
