Updated form for RAP assistive communication/speech pathology items

The Department of Veterans’ Affairs (DVA) has revised the Request for Assistive Communication Devices and/or Speech Pathology Apps/Software form, under the Rehabilitation Appliances Program (RAP).

The changes include providing additional clinical justification such as:

  • medical and speech pathology diagnoses and the clinical need for the device;
  • the client’s level of competency with technology; and
  • the functional benefit to the client from the use of the device.

The D1382 Request for Assistive Communication Devices and/or Speech Pathology Apps/Software form can be found on the DVA RAP Forms webpage. Assessing health providers should ensure that the most up to date forms are being used.
