Information to help veterans understand the support services available through participation in the Wellbeing and Support Program.

What is the Wellbeing and Support Program

The Wellbeing and Support Program is a pilot for the provision of an intensive and supportive case management service for veterans requiring additional support, with a focus on transitioning members.

Each veteran in the pilot will be supported by a contracted specialist case manager who will do all of the following:

  • work with you to identify supports and services you need
  • coordinate your clinical care
  • facilitate access to DVA and community services
  • help simplify your dealings related to your DVA entitlements

Your case manager will work closely with DVA to ensure you are receiving the appropriate level of assistance from DVA, your treating practitioners and community supports.

What does the Wellbeing and Support Program provide

The Wellbeing and Support Program will provide participants with all of the following:

  • greater coordination and improved communication between your treating practitioners and community supports
  • dedicated support by the Wellbeing and Support Program team to assist with your DVA enquiries and connect you to eligible DVA services
  • dedicated assistance from DVA Medical Advisors to help connect you with treatment providers and obtain the right level of support

Who is eligible for Wellbeing and Support Program assistance

Any veteran identified as potentially benefiting from the additional support offered by the Wellbeing and Support Program can be considered for participation in the Program. Veterans can be referred to the Program by DVA staff members, Defence transition support services, external Rehabilitation Providers and ex-service organisations/representatives.

What does the Wellbeing and Support Program include

The Wellbeing and Support Program will help identify areas of your life that you wish to improve and, with the assistance of a dedicated Case Manager, create a plan and activities to help you reach these goals.

What is the difference between a Wellbeing and Support Program and a Rehabilitation program

The difference with the Wellbeing and Support Program is that:

  • You will, where possible, be given the opportunity to select your case manager from up to three contracted specialists identified to have the experience and qualifications considered best suited to assist you with your individual needs and circumstances.
  • You will be supported by a dedicated Wellbeing and Support Program team who will work more closely with your case manager. This will help to identify gaps in your DVA services and connect you to eligible DVA services and services available in the community, in a quicker way.
  • You will receive regular communication and case conferences to ensure your case manager, treating practitioners and support services are offering the best level of support for you.
  • Your case manager will have greater capacity to provide outreach services such as conducting appointments in your home, and accompanying you to appointments and activities where you require support.

What is my role in the Wellbeing and Support Program

By participating in the Wellbeing and Support Program, you will have an active role in determining what goals are important to you and what areas you would like to focus on.

The aim of the Wellbeing and Support Program is to develop your strengths and skills to improve your sense of wellbeing, engagement and empowerment.

What if I no longer wish to participate in the Wellbeing and Support Program

As the Wellbeing and Support Program is voluntary, you are able to withdraw your consent to participate at any time.

If you do not wish to continue with the program, you are encouraged to contact the Wellbeing and Support Program team to discuss how the Program or other DVA services may better support you.

DVA will meet the full reasonable cost of your approved Wellbeing and Support Program.
