What is the Coordinated Veterans' Care (CVC) Program
The Coordinated Veterans' Care (CVC) Program uses a proactive approach to improve the management of chronic conditions and your quality of care. This will allow you to become actively involved in your health and wellbeing.
As a team-based program, you, your general practitioner (GP) and a Nurse Coordinator (NC) will work together as a Core Team. The idea of this Core Team is to make sure that the care you get is the care you need.
As a member of this Core Team, you will work to:
● create an individual Care Plan to meet your health needs
● support you in achieving your health goals
● help educate and guide you to understand your health; and
● manage your ongoing care
You may also have a larger Care Team to support your unique goals. This Care Team can include, along with your Core Team:
● other health professionals such as pharmacists, physiotherapists, mental health specialists
● community organisations such as a Ex-Service Organisations
● family and friends
Who can access it
If you think you would benefit from participating in the CVC Program, first check you are eligible. You will need to have either:
A DVA-accepted mental health condition means the condition has been accepted by us as being related to your military service.
If you are eligible, talk to your GP about the CVC Program.
If you are not eligible, you can still get mental health treatment at no cost to you through Non-Liability Health Care.Back to top
How you access

How much you pay
There is no charge to participate in the program. If you are a Veteran White Card holder you may need to pay for any treatment, as part of your Comprehensive Care Plan, not related to an accepted condition on your card or funded under NLHC or PAMT.Back to top
Things you should know
- You may not be eligible for the program if you:
- are taking part in a similar program,
- living in a Residential Aged Care Facility,
- your condition is considered terminal (at the time of enrolment), or
- you have a Veteran White Card without an accepted mental health condition.
