Forms

• Authority to release personal information (Veterans' Home Care) (D406)
  Only for use by war widows/widowers and veterans of the Austalian Defence Forces who may transfer from Home and Community Care (HACC) services to Veterans' Home Care.
  
  
• Application for waiver of Veterans' Home Care co-payments (D700)
  Only for use by war widows/widowers and veterans of the Australian Defence Forces who have been approved by the Veterans' Home Care Regional Assessment and Coordination Agency to receive basic home care services and who wish to apply for waiver of co-payments due to financial hardship.
