Appendix A - Form D9166: Request for thickeners (PDF 135 kB)

This form is used by Speech Pathologists requesting a subsidy for thickeners and thickened products for eligible veterans and war widow(er)s. It includes procedures for obtaining thickeners, and explanatory notes.
