These notes explain the procedures to be followed when providing health care services to eligible members of the veteran and defence force community.

Appendix A – Form D9165: Request for nutritional supplementation (PDF 132 kB)

This form is used by Dietitians requesting a subsidy for nutritional supplements for eligible veterans and war widow(er)s. It includes the procedure for obtaining supplements, and explanatory notes.
