Prior approval information for dentists
Note: You must obtain prior approval for some services before you commence treatment. The need for prior approval depends on the beneficiary's entitlement and on the type of treatment proposed. Refer to the fee schedule for items requiring prior approval.
DVA recently undertook a review of prior approval items, see changes effective 1 September 2012 for more information.
To request prior approval from DVA, please send a completed Dental Request (Form D986) or a request on your letterhead to:

For assistance when requesting prior approval for dental implants go to the Dental Implant heading in the navigation pane on the left of the screen. There are prior approval processes and templates to assist when requesting prior approval.
Referral to dental specialists
Prior approval is not required for initial dental specialist consultations excluding orthodontic and prosthodontic specialists.
The LDO should ensure that the dental specialist will accept DVA treatment arrangements and dental specialists' fees. If there is any question about specialist acceptance, please confirm with the dental specialist or contact the DVA office on the above phone numbers.
