Documentation requirements for the Exceptional Case Unit
Application for Exceptional Case Status
When submitting an Application for Exceptional Case Status (application) for a veteran or war widow/widower, the only additional information required by the Exceptional Case Unit (ECU) to be included with an application is:
- Current treatment / care plan
Please note that progress notes are not required to be sent with an application. Send specialist assessments and supporting documentation that are relevant to the application.
Sending Exceptional Case Forms by fax
An application and the attachments, if required, can be faxed to the ECU.

All other Exceptional Case Status forms or Palliative Care forms can also be faxed to the ECU.
Posting Exceptional Case Forms
Post original signed applications and attachments to:

The ECU requires the original signed exceptional case status forms to be able to assess exceptional case status. Photocopies of the exceptional case status forms are not acceptable as they may be difficult to read and interpret.
