Information about the claiming voucher spreadsheet

The Department of Veterans' Affairs Community Nursing Program has developed a more efficient method to submit community nursing paper-based claims.

If you adopt this method, it removes the requirement to complete individual community nursing service vouchers (D1083) for each veteran or war widow/er (entitled person).

All information required for a 28-day claim period can now be provided in a table format, following the attached Claiming Voucher Spreadsheet and Guide.

This method is only for printed text (either as a computer printout or typewritten page). It will not be accepted in handwriting.

Each table row represents a single 28-day claim period for a single entitled person. Each column represents information required for claiming. Up to 40 28-day claim periods can be batched together in the spreadsheet.

You must still complete and sign the Claim for Treatment Services Voucher (D1217) and attach it to your Claiming Voucher Spreadsheet(s) when sending.

Alternatively, if you would like to develop your own print-out generated by your practice management software, please refer to the format instructions.
