Information on Claiming Voucher Spreadsheet
The Department of Veterans' Affairs, Community Nursing Program, is pleased to announce a more efficient method to submit Community Nursing paper-based claims. This method has been identified as part of the business analysis phase of the development of the Veterans' Nursing Electronic System (VNES).
If you would like to adopt this new method, it will remove the requirement to complete individual Community Nursing Service Vouchers (D1083).
All information required for a 28 day claiming period can now be provided in a table format, following the attached layout and guidelines (see format instructions (Microsoft Word 48Kb). Currently this method is only for printed text (either as a computer print-outs or type written page). It will not be accepted in hand writing.
Each table row represents a single 28 day claim period for a single veteran or war widow. Each column represents information required for claiming. Up to 40 claim periods can be batched together.
The Claim for Treatment Services Voucher (D1217) must still be completed, signed, attached and sent with your claim sheet(s).
A Claiming Voucher Spreadsheet (Microsoft Excel 114Kb) has been developed for you to use - You will need to save the Community Nursing Claiming Spreadsheet to your local disk before attempting to open it.
Alternatively, if you would like to develop your own print-out which can be generated by your practice management software, please refer to the format instructions (Microsoft Word 48Kb).
