I am writing with regard to the Vietnam Veterans' Study that we are currently conducting. As of today, we have not yet received your completed questionnaire.
The information collected in this survey will be used by the Department of Veterans' Affairs to ensure that the care and support that they provide to Vietnam Veterans and their families is appropriate.
I am writing to you again because of the significance each questionnaire has to the usefulness of this study. It is very important that you return your completed questionnaire if the results are to accurately represent the health of all Vietnam Veterans and their families.
In the event that your questionnaire has been misplaced, a replacement is enclosed. Could you please return your completed questionnaire as soon as possible.
