I am writing with regard to the current survey we are conducting relating to Vietnam Veterans' health and the health of their families.
All surviving Australian Vietnam Veterans on the nominal roll should have received a questionnaire in the past few weeks. If you have already returned your questionnaire, thank you for your reply. If not, please do so as soon as possible. It is very important that you return your completed questionnaire if the results are to accurately represent the health of Vietnam Veterans and their families.
If you did not receive a questionnaire please contact our freecall help line on 1800 555 048 so that we can record your details and send you a copy to complete. If you did receive a questionnaire but have misplaced it, call the help line and we can send you another copy.
We would like to remind Veterans that the Department of Veterans' Affairs will not be provided with data that can identify individuals and answering the questionnaire will not threaten any payments you may receive from the Department. The barcode on the questionnaire is simply to allow AGB McNair to follow up and record returns against the mailing list.
The date for receipt of questionnaires has been extended until the end of June.
