What is the purpose of the study?
To learn about your health and that of your partner and children, so that the approaches to the care and support of the Vietnam Veteran community are appropriate. Your assistance will make this possible.
Who is included in the study?
All surviving Vietnam veterans, based on 'The Nominal Roll of Vietnam Veterans', are being asked to participate in this study.
How confidential is the information I give?
AGB McNair P/L are contracted to conduct the study and to issue a report on their findings. No individual respondents will be identified. Any benefits you receive from the Department of Veterans' Affairs will not be threatened by your answers to the questionnaire. When the study is completed, the Australian Institute of Health and Welfare will safeguard the privacy of your responses.

Simply tick the boxes as directed, starting at Part A: Medical History. Where the questions ask 'How many', insert the number in the box or in the space as directed. Where the questions request additional information please be as brief, precise and clear as possible in giving your answer.
