Getting to the DVA Forms System

Forms system. Once in the DVA Forms system, choose Form Number
D3057 by selecting the Numeric option. D3057 can then be
selected from the drop down list of forms.
- The information sought on the application form is required
to assess your eligibility for a Gold Card under the Veterans'
Entitlements Act 1986.
- You may have previously provided some or all of the information
requested by this form. By providing it again however, you will
help the Department to quickly make a decision on your application.
- Please notify the Department if you change your address.
- You should be aware that there are penalties for making false
or misleading statements.
