Lodging my claim

If you wish to lodge a claim for pension or benefits from DVA, you can write to us, or phone us on 1300 55 1918 and ask for a claim form to be sent to you. You can also ask your local ex-service organisation to give you a form, or download it from the Forms section of this website.

Help

If you are unsure of which form to complete, or you find any difficulty with completing the form, you should get help. You can ask your local ex-service organisation as they often have pension officers and advocates who are specially trained to help you. Or you can visit or call your local VAN office.

Liability or pension reassessment

Form

If you wish to claim benefits for a condition relating to service on or after 1 July 2004, you need to complete the D2051  Claim for Liability and/or Reassesment of Compensation (PDF 156kB). 

For claims relating to service before 1 July 2004, you need to complete the form D2020 Claim for Rehabilitation and Compensation (PDF 184kB).

Disability Pension

Form

To apply for acceptance of a disability as related to service, or to apply for an increase in your current rate of disability pension, you need to complete the form D2582 Claim for Disability Pension and/or Application for Increase in Disability Pension (PDF 164kB).

Service Pension

If you are a veteran and wish to apply for a service pension, you will need to complete forms D0503 Claim for Service Pension Part A – Eligibility and D0648 Claim for Service Pension or Income Support Supplement Part B – Income and Assets.

If you are the partner, widow or widower of a veteran and wish to apply for service pension, you will need to complete forms D0504 Claim for Service Pension by a Partner or Widow(er) Part A – Eligibility and D0648 Claim for Service Pension or Income Support Supplement Part B – Income and Assets.

War widow's pension

Form

To apply for a war widow's or widower's pension or an orphan's pension relating to the death of a veteran before 1 July 2004, you need to complete the form D2663 Claim for Pension by a Widow, Widower or other Dependant of a Deceased Veteran (PDF 184kB).

To claim compensation as the partner of a deceased member or former member whose death relates to service on or after 1 July 2004 you need to complete the claim form D2053 Claim for Compensation for Dependants of Deceased Members and Former Members (PDF 195kB).

Income Support Supplement

To claim income support supplement you need to be an Australian war widow/widower; and complete forms D0529 Claim for Income Support Supplement Part A - Eligibility and D0648 Claim for Service Pension and Income Support Supplement Part B – Income and Assets.
